NOTE 1: Smoking is the major cause of cancer of the bladder. More than 85% of all bladder cancers originate from transitional epithelium. Usually the first and only sign of bladder cancer is presence of blood in the urine. Stones in bladder can also cause bloody urine. Other symptoms include frequent urination, strong urges to urinate and discomfort or burning during urination.
NOTE 2: There are several methods of surgery for removal of cancer and most of them result in impotency in males and physiological changes like alteration in sensation and potential for orgasm in females.
NOTE 3: Drinking about 2-3 litres of fluids like water, soda, etc. each day can cut the risk of bladder cancer to half. The liquids probably flush away the carcinogens and keep urine diluted so toxins make less contact with the bladder wall.
NOTE 4: Some patients come with the following complaints in connection with the urinary bladder:
- Increased frequency of urination – more than eight times during 24 hours.
- Waking up at night two or more times for urination.
- Strong, sudden urge for urination – if not attended, results in voiding of urine.
- Involuntary leakage of urine – small or large.
The explanation is simple. Body absorbs liquids every time a person eats or drinks. The kidneys filter out wastes in the form of urine to the bladder. When the bladder is full, nerves in it, send a signal to the brain and it produces an urge for urination. When the person goes to the bathroom for this, the brain sends a message to the large bladder muscles to contract, and to the sphincter muscles surrounding the urethra to relax and let the urine through (see the diagram). This means a person should urinate only when he wants to.
Problems arise when the muscles are too weak or too active or nerve cells do not function properly or if the bladder becomes overactive. Bladder can become overactive from minor nerve damage during surgery or during giving birth to a child. In such cases, there is a sudden urge for urination though there is little amount of urine in the bladder. Men can have problem due to an enlarged prostate. In women, the muscles around the urethra can become weak during pregnancy or while giving birth to a child or during menopause. A small amount of urine may escape on coughing, sneezing, laughing, exercising or lifting heavy weights.
Another reason can be infection of the urinary tract. In such cases, a burning sensation is felt during urination and often a small amount of urine is passed in drops.
Unlike common belief, these problems are not caused due to aging. These can be treated and cured. For this, refer to the chapter Urine.
